The University of Pennsylvania Health System and RightCare Solutions, Inc. Announce Expanded Partnership to Reduce Readmissions

April 4, 2013

Philadelphia, Penn. –April 4, 2013 – The University of Pennsylvania Health System and RightCare Solutions, Inc., an evidence‐based technology organization specializing in hospital care transitions and readmission management, today announced an expanded partnership with the goal of improving care coordination and reducing readmissions. The partnership with RightCare will extend across all Penn hospitals including the Hospital of the University of Pennsylvania, Penn Presbyterian Medical Center, and Pennsylvania Hospital. By leveraging RightCare’s Discharge Decision Support System (D2S2), the health system’s staff of nurses, clinical resource coordinators, and social workers will utilize evidence‐based point‐of‐care tools to improve and enhance patient outcomes.

“We were encouraged by early D2S2 outcomes data, so we made the decision to expand our partnership with RightCare into all three of our acute‐care hospitals,” said Dr. PJ Brennan, Chief Medical Officer of the Penn Health System. “Through this collaboration we are looking forward to exploring the clinical and workflow benefits that the software will deliver.”

“Hospitals are accustomed to multi‐year, expensive IT projects,” said Roy Rosin, Chief Innovation Officer, Penn Health System. “RightCare, on the other hand, is swift and nimble in terms of the software implementation. We’re excited about the potential and the opportunity to test a promising  new approach rapidly.”

“Financial pressures due to readmissions are significant, and hospitals across the country are seeking scientifically‐driven, evidence‐based tools that identify high‐risk patients, optimize workflow, and automate care transitions,” said Eric Heil, Co‐Founder and CEO of RightCare. “We are excited to provide this foundation for Penn’s leading readmissions reduction efforts.”
