RightCare Solutions, Inc. Assesses 30,000th Patient for Risk of Readmission

July 17, 2013

Philadelphia, Penn. – July 17, 2013 – In its mission to lower potentially preventable 30- and 60-day readmissions, RightCare Solutions, Inc., an evidence-based technology organization specializing in hospital care transitions and readmission management, announced today that it has screened its 30,000th patient through the D2S2 Care Transitions software platform. Pioneered through over a decade of academic research led by Dr. Kathryn Bowles, Professor of Nursing at the University of Pennsylvania School of Nursing, D2S2 (“Discharge Decision Support System”) was developed to predict which patients require post-acute care and who are thus at higher risk of readmission.

“When I began studying the root causes of readmissions back in the late 1990s, never in my wildest dreams did I think that I’d see this research developed into a commercially available software platform that can achieve this level of scale so quickly,” said Dr. Bowles. “Workflow is everything, and what the RightCare team has been able to accomplish in such a short amount of time is astonishing.”

Reducing readmissions using the D2S2 software platform begins at admission, generating a RightReferTM score that designates not only those patients who are high-risk but also which specific post-acute care interventions are most appropriate. By accessing D2S2 via their Electronic Health Record (EHR) or Case Management system, care managers leverage workflow tools that help prioritize their time and optimize their staffing needs. Personalized care plans can be developed for each patient based on individual risk factors, and electronic referrals can be transmitted to the right agency; at admission and within a software platform that is interoperable with existing IT systems.

“It takes less than eight weeks to integrate our cloud-based software into a health system’s EHR or Case Management system,” said Mrinal Bhasker, chief technology officer at RightCare Solutions. “Through the use of big data modeling techniques, this robust data set has helped us glean key insights that meet the specific dynamics and needs of each individual hospital we serve.”

“We are committed to building an enduring health technology company that improves the lives of patients and those who care for them,” said Eric Heil, co-founder, president, & CEO of RightCare Solutions. “I’m eagerly anticipating the day when we announce the thirty millionth patient assessed using our peer-reviewed care transitions software platform.”
